Gemma is Skye Riley’s old friend and someone Skye reconnects with after years apart. Their friendship is complicated because Skye’s addiction and career problems had damaged their relationship. The big twist is that Gemma isn’t actually with Skye during the events we see. After Skye calls her for help, the movie later reveals that those interactions were part of the Smile Entity’s manipulation. The Entity uses visions and hallucinations of people Skye trusts—including Gemma—to make Skye question what is real. The most important reveal is that Skye’s entire attempt to escape the Smile curse is basically being manipulated by the Entity. What looks like Gemma helping Skye, escaping the facility, and getting to safety is actually part of the hallucination. By the ending, Skye is back onstage at her concert, where she realizes far too late that what she experienced wasn’t real. Then the Entity appears to her as her mother and ultimately takes control of the situation. Skye is killed onstage in front of the audience, and the Smile curse passes to one of her fans who witnesses what happens. So Gemma isn’t secretly the villain—the movie uses her as part of the Entity’s deception.
