Erm actually his was a birthmark it just wasn’t fully developed until after the final selection same with Yorrichi which Tanjiro is a descendant of which means Tanjiro and Muichiro are somewhat related up along the famiy tree since Muichiro is a descendant of Kokushibo A.K.A Yorrichi’s brother.
